Blended Case Management

Lakeshore Community Services, Inc. provides Blended Case Management for adults 18 years and older who have serious and persistent mental illness. This program operates 24 hours a day, 365 days a year. The Blended Case Management Program aims to provide flexible, racially and culturally appropriate, person-directed services that assure that individuals succeed in their chosen environments, increase their independence and in turn maintain a sense of identity, dignity and self-esteem. Blended Case Managers provide service coordination which includes assisting individuals with treatment planning, developing a system of supports, accessing appropriate services and linking to other community resources. Specifically, Blended Case Managers can support individuals by making referrals for services related to mental health care, housing, income and benefits, medical care, drug and alcohol treatment, activities of daily living, social support and education and work.
